MSU Camp Complete Details!!

Thank you for registering for our 2025 Falcon Athletics Field Hockey Camp at Montclair State University! We hope that this camp will be an unforgettable and exciting opportunity for your camper to improve their skills.  Each camper will have the opportunity to work with some of the top coaches and players in the game!  We are so excited to start camp! 


Attached you will find our Pre Camp Packet. The Pre Camp packet is designed to help you prepare for your upcoming camp. Please read all documents carefully, as they contain all the forms, important information, schedules, checklists, and tips you need to set your camper up for a smooth, successful camp experience.  Link sharing is turned on for all documents, please check your network sharing options if you are unable to open them.  
 
Each camper will need to print & bring the following forms to camp on the first day.  You may NOT mail forms in, and we can NOT accept scanned copies. 
  • Complete the health history and release form and bring a printed copy with you to check in on the first day of camp! **If you are doing both weeks you will need to bring this form both times please**

  • For the authorization of medication administration form, please see this note with instructions for different types of prescriptions.
